HOME HEALTH CARE
Care is provided in the home by involving the patient, family and physician to develop and implement a plan of home care.
Services Provided: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aide, Homemaker and Medical Social Worker.
VNA is a certified home health agency. Home visits may be reimbursed through Medicare, Medicaid, or Private Insurance. For services that cannot be billed to these sources, charges to patients are based on a sliding fee scale according to income and need.
VNA is the only agency in Jones County that holds a State and County funded grant to help care for patients in their home using a sliding scale. Services provided under this grant include skilled nursing, home care aide, and homemaker services.
PUBLIC HEALTH
Immunizations: We have the Vaccines for Children (VFC) Program which provides vaccines to children who are underinsured, American Indian/Alaskan Native, enrolled in Medicaid and no health insurance.
Senior Clinics are held throughout the county in different towns. Services provided are blood pressure screening, blood sugars and cholesterols (fasting) and foot care.
Disease Investigation and Follow-up and Emergency Response/Preparedness
We have collaborative relationships with several partners in the county: Board of Health, Board of Supervisors, Empowerment, EMA, Jones County Youth Coalition, Jones Regional Medical Center, St. Luke’s Family Health Center and many others.
